Palliative Care Nurse Practitioner Opportunity

We are seeking a skilled and compassionate Nurse Practitioner to join our team at Ascend Hospice & Palliative Care PA. As a Palliative Care Nurse Practitioner, you will play a vital role in providing high-quality care to patients and their families.

Responsibilities:
  • Conduct home visits to patients receiving palliative care, ensuring compliance with local, state, and federal regulations.
  • Communicate visit findings to the interdisciplinary team, including the National Director of Palliative Care.
  • Facilitate access to supportive care models, such as hospice, rehabilitation, and ongoing aggressive treatment.
  • Collaborate with the interdisciplinary team to develop patient care plans.
  • Maintain accurate patient records, charts, and documents.
  • Contribute to the development of clinical practice guidelines and standards for quality palliative care.
  • Ensure collaboration with patients, caregivers, medical directors, and members of the interdisciplinary team.
Requirements:
  • Master's Degree in Nursing
  • Nurse Practitioner's license in PA
  • 2-4 years of experience working in hospice or a related field
  • Palliative care experience preferred
  • Valid driver's license and reliable transportation
  • Willingness to travel
