Key Responsibilities

  • Collaborate with healthcare providers and caregivers to develop personalized care plans that cater to individual patient needs.
  • Connect patients with community resources, empowering them to maintain optimal health and well-being.
  • Promote timely access to essential care services, highlighting the importance of preventative measures.
Requirements
  • Minimum 2 years of experience in social services or a related field.
  • Associates degree required; bachelor's degree preferred.
  • Bilingual Spanish proficiency is strongly desirable.
