Job Overview

Solace Hospice & Palliative Care, Inc. is dedicated to providing exceptional care and support to patients and their loved ones during the end-of-life journey.

We are seeking a compassionate and skilled Hospice CNA to join our team and provide personalized care and attention to patients in their homes.

This role offers an estimated annual salary of $55,000 - $65,000, depending on experience and qualifications.

Responsibilities
  • Provide personal care services, including bathing, grooming, and dressing, as needed;
  • Assist with feeding, hydration, and medication management;
  • Monitor vital signs and report any changes to the primary care nurse;
  • Support patients with ambulation and exercise, as instructed by the primary care nurse or therapist;
  • Keep patient living areas clean and orderly, as assigned;
  • Fulfill the obligation of requested case assignments;
  • Attend interdisciplinary group meetings;
  • Maintain and upgrade professional knowledge and practice skills through continuing education and in-service classes;
Requirements
  • Certified Nursing Assistant (CNA) certification;
  • High school diploma or equivalent required;
  • Ability to work effectively in a team environment;
  • Excellent communication and interpersonal skills;
  • Ability to lift 25 pounds and stand for long periods;
  • Valid driver's license (if applicable).
