Job Summary

We are seeking a highly skilled and compassionate RN Chronic Condition Manager to join our team. As a key member of our healthcare team, you will be responsible for developing and implementing personalized care plans for patients with chronic health conditions. Your expertise and dedication will play a critical role in improving health outcomes, enhancing quality of life, and reducing healthcare costs.

Key Responsibilities
  • Assess patient needs and develop individualized care plans to address chronic health conditions
  • Collaborate with healthcare providers, patients, and families to ensure seamless care coordination
  • Provide education and support to patients and caregivers on disease management and self-care
  • Monitor patient progress and make adjustments to care plans as needed
  • Communicate effectively with healthcare teams, patients, and families to ensure timely and efficient care
Requirements
  • Current unrestricted RN license in the state of practice
  • Minimum 2 years of direct clinical care experience
  • Excellent communication and interpersonal skills
  • Basic typing and computer skills
  • Previous disease management or case management experience preferred
