About the Role:

We are seeking a skilled Case Manager RN to join our team at Carilion Clinic. As a key member of our healthcare team, you will play a vital role in ensuring the best possible outcomes for our patients.

Key Responsibilities:
  • Collaborate with healthcare professionals to develop and implement individualized care plans for patients.
  • Conduct thorough assessments of patients' needs and develop strategies to address any barriers to care.
  • Coordinate with healthcare providers, insurance companies, and community resources to ensure seamless transitions of care.
  • Provide education and support to patients and their families to promote healthy behaviors and self-management of chronic conditions.
  • Monitor patients' progress and adjust care plans as needed to ensure optimal outcomes.
Requirements:

To be successful in this role, you will need:

  • A current RN license in Virginia.
  • At least 3 years of experience in a clinical healthcare setting, with a focus on case management or patient care coordination.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Strong analytical and problem-solving skills, with the ability to think critically and make sound decisions.
  • Ability to work independently and as part of a team, with a focus on delivering high-quality patient care.
