The Role
As a Clinical Care Specialist on our Acute Rehab Unit, you will utilize the nursing process to facilitate patient care by performing nursing assessments, formulating care plans based on diagnoses, and collaborating with the team to create and implement plans of care.
Responsibilities
- Nursing Assessments: Perform thorough assessments to identify patient needs and develop individualized care plans
- Care Planning: Collaborate with the team to create, implement, and modify care plans based on patient diagnosis and progress
- Patient Education: Educate patients and their families on health conditions, treatments, and self-care strategies
- Team Collaboration: Work closely with interdisciplinary teams to ensure seamless care coordination and effective communication
Qualifications
To be successful in this role, you must have:
- An Associate Degree in Nursing (ADN) or Bachelor's Degree in Nursing (BSN)
- A valid Florida Registered Nurse License
- BLS certification
- CRRN preferred; Quality Indicator Measure cert within 6 months of hire
- Ability to read, write, speak, and understand English
Compensation and Benefits
As a valued member of our team, you can expect a competitive salary range of $62,000 - $80,000 per year, depending on experience. We also offer a comprehensive benefits package, including medical, dental, and vision coverage, as well as opportunities for professional growth and development.
