We are seeking a compassionate and skilled Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) to join our team at AccentCare, Inc. as a Home Health LVN/LPN. In this role, you will provide essential nursing services to patients in their homes while maintaining accurate medical records.

Key Responsibilities:
  • Provide nursing visits according to the treatment plan prescribed by the physician
  • Document patient progress and maintain accurate and complete medical records
  • Collaborate with the healthcare team to ensure seamless care delivery

What We Offer:

Qualifications:
  • One year of clinical experience and one year of home health experience
  • Graduate of an accredited practical or vocational nursing program
  • Currently licensed as a Practical Nurse or Vocational Nurse in the state of operation
  • Valid CPR certification
