Responsibilities

This role will involve administering physical therapy treatment plans, conducting patient assessments, and developing personalized care plans. You will also be responsible for maintaining accurate records, communicating effectively with patients and families, and collaborating with interdisciplinary teams. Your expertise will help patients regain independence, manage chronic conditions, and live healthier lives.

Requirements

To succeed in this role, you will need:

  • An Associate degree in Physical Therapy from an American Physical Therapy Association accredited program.
  • A current state license to practice Physical Therapy.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Proof of current CPR certification.
  • A minimum of two years of experience as a Physical Therapy Assistant, preferably in a home healthcare setting.
