Sanctuary Hospice is seeking a Full-Time Registered Nurse Case Manager (RNCM) who is eager to positively impact the lives of patients and their families in the surrounding communities.


WORK SCHEDULE:
MON-FRI 8AM - 4:30PM


We are in search of a dynamic, empathetic, and meticulous Registered Nurse (RN) to serve as a Hospice Case Manager for our patients.

Our Hospice RN Case Managers are responsible for planning, organizing, and directing hospice care by utilizing the nursing process, which includes assessment, planning, interventions, implementation, and evaluation. They will engage effectively with patients, caregivers, families, and other interdisciplinary team members while upholding the standards of professional nursing and clinical excellence.

Key Responsibilities:

Empower Through Engagement:

Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch that reflects our commitment to exceptional service.


Collaborative Care Champion:

Work closely with our Clinical Director to evaluate the eligibility and appropriateness of clients for our specialized hospice services. Your expertise will guide us in delivering the right care at the right time.


Nurturing Direct Care:

As an RN Case Manager, you will not only coordinate care but also provide comfort and support for patients, ensuring their comfort and quality of life remain paramount.


Nursing with Heart:
Utilize your nursing skills to deliver high-quality care grounded in proven principles and techniques. Your dedication will help alleviate pain and provide solace to those under our care.

Educator and Advocate:

Empower patients and families through education, encouraging their active involvement in creating personalized care plans that align with their goals.


Call of Compassion:
Participate in shared call duties, demonstrating our commitment to being present when it matters most.

Qualifications:
- Hold a current unencumbered license as a Registered Nurse.
- Minimum one (1) year of experience as an RN in a medical surgical/acute care setting; hospice experience is a plus.
- Must be proficient in computer skills, including typing and various programs, with a background in EMR.
- Maintain current CPR certification as required by state regulations.
