Position Summary:
As a Home Care Nurse, you will be responsible for the effective management and delivery of direct nursing care to participants in various settings, utilizing the nursing process and adhering to established standards of practice.
Key Responsibilities:
1. Review and implement provider orders.
2. Conduct comprehensive nursing assessments, including physical and psychosocial evaluations.
3. Engage participants and their families based on individual needs and capabilities.
4. Deliver personalized care in home settings, including skilled nursing services.
5. Address the cultural and diverse needs of participants during care delivery.
6. Monitor and assess therapeutic interventions.
7. Collaborate in the development and ongoing review of care plans.
8. Delegate and accept care responsibilities within the healthcare team as appropriate.
9. Identify emergencies and initiate appropriate nursing interventions.
10. Ensure timely response to participant needs.
11. Actively participate in interdisciplinary team meetings to address participant needs.
12. Document all aspects of participant care accurately in medical records.
13. Assist in training and orienting new nursing staff as needed.
14. Participate in nursing on-call rotations.
15. Complete necessary reports related to falls, infections, grievances, and incidents.
16. Engage in site-specific committees and meetings as required.
Qualifications:
Education:
Graduate of an accredited nursing program.
Licensure/Certifications:
Current RN license in Massachusetts. CPR and Alzheimer's certification (or willingness to obtain within 60 days of hire) are essential.
Experience:
A minimum of two years of recent experience in direct care for adults or chronically disabled individuals, with at least one year of experience caring for elderly populations. Experience in ambulatory or home care settings is preferred.
Other Requirements:
Reliable transportation is necessary.
