About the Role

We offer an exciting opportunity for a Physical Therapist to work with our home health team, providing expert care and support to patients in their own homes. As a member of our team, you will have the chance to make a meaningful difference in the lives of our patients and their families.

  • Salary: $65,000 - $85,000 per year, commensurate with experience.

In this rewarding role, you will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. You will work closely with our multidisciplinary team to develop and implement effective treatment plans, improving and enhancing the well-being of our patients.

Responsibilities
  • Evaluate patients' functional status and physical therapy needs, working closely with physicians to develop comprehensive therapy plans.
  • Provide expert physical therapy interventions, helping patients achieve optimal functionality and independence.
  • Observe and report patients' responses to treatment, making adjustments as needed to ensure optimal outcomes.
  • Collaborate with healthcare professionals to educate patients and their families about treatment plans, functional limitations, and progress towards goals.
Requirements
  • Current Physical Therapy licensure in the state of practice.
  • Current CPR certification required.
  • A valid driver's license and reliable transportation.
