Registered Nurse - Long Term Care
The Long Term Care RN is responsible for overseeing and delivering quality care to patients residing in Long Term Care and Assisted Living Facilities. This role requires the nurse to demonstrate sound clinical judgment and effective critical thinking skills while coordinating patient care under supervision.
Key Responsibilities:
- Employs the nursing process to assess, plan, implement, and evaluate patient care within a Long Term Care or Assisted Living setting.
- Conducts focused assessments to identify individual patient needs.
- Monitors and evaluates signs and symptoms that may indicate physiological or psychological changes in the patient's health status.
- Gathers, analyzes, and interprets data from the healthcare team, documenting actual and potential nursing diagnoses.
- Develops and documents the patient's care plan, incorporating identified nursing diagnoses, anticipated patient outcomes, and selected nursing interventions, along with responses to those interventions.
- Executes interventions based on established priorities, care plans, and facility policies to meet patient outcome standards.
- Ensures a safe, comfortable, and therapeutic environment for patients.
Qualifications:
- Valid Registered Nurse License in the practicing state.
- Adheres to all relevant professional standards of nursing practice.
- Completion of Amergis' Competency program, if applicable.
- Current CPR certification, if applicable.
