Responsibilities- Conduct regular wellness visits for all residents to identify and address any health concerns.
- Assess, monitor, and follow up with residents who have experienced any falls, accidents, or incidents, documenting and reporting as required.
- Maintain accurate and up-to-date resident wellness files, ensuring compliance with Sunrise policies and federal and state regulations.
- Collaborate with the Resident Care Director and community team to promote infection control standards, including resident testing, training, and skills checks.
- Regularly monitor each resident's medication and treatment profile to ensure accurate administration and documentation.
- Participate in Sunrise Quality Assurance Meetings and Clinical Meetings to contribute to quality improvement initiatives.
- Graduate of an approved college or school of nursing.
- Current state license as a professional Registered Nurse (RN).
- Minimum of one (1) year experience in home health, assisted living, hospital, or long-term care environment.
- Demonstrated knowledge of nursing practices, techniques, and methods applied to health and wellness resident care, and federal, state, and local regulations.
- Ability to handle multiple priorities, possess excellent communication and organizational skills, and demonstrate good judgment, problem-solving, and decision-making skills.
