Position Overview

The MDS Coordinator plays a crucial role in overseeing and facilitating the accurate and timely completion of interdisciplinary MDS Assessments, CAAs, and Care Plans, ensuring compliance with CMS RAI Manual Regulations and all relevant laws and standards set forth by Life Care Center of America.

Qualifications and Experience

  • An associate's or bachelor's degree in nursing from an accredited institution.
  • A valid and active Registered Nurse (RN) license in the applicable state.
  • A minimum of two years of nursing experience, with a preference for experience in geriatric care.
  • Certification in CRN C (clinical compliance).
  • CPR certification is required upon hire or must be obtained during orientation, and must remain current throughout employment.

Key Responsibilities

  • Possess advanced knowledge in nursing practices.
  • Make independent decisions when necessary.
  • Understand and apply the laws, regulations, and guidelines governing operations in a post-acute care environment.
  • Implement and interpret departmental programs, goals, objectives, policies, and procedures.
  • Exhibit proficiency in all competency areas, including patient rights, safety, and sanitation.
  • Maintain professional relationships with colleagues and vendors.
  • Uphold confidentiality regarding all proprietary and sensitive information.
  • Adhere to company policies, including those related to harassment and compliance.
  • Demonstrate integrity and professionalism by following Life Care's Code of Conduct and completing required compliance training.

Essential Duties

  • Coordinate and assist in the timely and accurate completion of interdisciplinary MDS Assessments, CCAs, and Care Plans in accordance with CMS RAI Manual Regulations.
  • Report any changes in patient conditions identified through MDS Assessments to the Director of Nursing.
  • Educate direct care staff on updates or changes to CMS RAI Manual or Skilled Nursing Facility Regulations that affect documentation.
  • Assist in reviewing the Interdisciplinary Comprehensive Care Plan.
  • Validate that all assessments are completed and accepted into the CMS QIES system prior to billing, and notify the Business Office of any delays.
  • Analyze CMS Reports to identify late submissions and develop strategies to prevent future occurrences.
  • Participate in Daily PPS Meetings, Monthly Triple Checks, and other relevant meetings as needed.
  • Fulfill the duties of a staff nurse as required.
  • Provide excellent customer service and maintain a positive attitude towards patients.
  • Assist in patient evacuation procedures when necessary.
  • Demonstrate reliable attendance and punctuality.
  • Utilize reasoning skills and sound judgment in all tasks.
  • Collaborate effectively within an interdisciplinary team.
  • Engage in physical activities such as sitting, standing, bending, lifting, pushing, pulling, walking, and reaching throughout the workday.
  • Communicate effectively in English, both verbally and in writing.
