Clinical Case Management Nurse

Position Overview
As a Case Management Nurse, you will play a crucial role in overseeing patient care and ensuring effective management of healthcare services. Your expertise will be essential in coordinating care plans and supporting our team in delivering high-quality services.
Location: Chatsworth
Schedule: 8:00 A.M. - 5:00 P.M.
Compensation: $50 - $55 with Full Benefits
Required Qualifications
- Active California Registered Nurse License
- Minimum of 5 years of experience in case management, preferably within managed care
- At least 2 years of leadership experience
- Experience in a managed care environment for approximately 2-5 years
- Familiarity with Medicare member case management
Key Responsibilities
- Maintain the Master Log for daily staff assignments
- Review and sign off on all completed care plans from the previous day
- Distribute completed care plans to Case Management Coordinators
Biweekly Responsibilities
- Process payroll for staff
Monthly Responsibilities
- Prepare health plan monthly reports
- Conduct health plan monthly meetings
- Facilitate team meetings
- Perform internal departmental self-audits
Quarterly Responsibilities
- Compile health plan quarterly reports
Annual Responsibilities
- Complete staff performance evaluations
Additional Duties
- Participate in health plan webinars and audits
- Maintain departmental tutorials, policies, and procedures
- Contribute to the development of new policies and procedures
- Engage in strategic planning initiatives
- Support the Case Management Department with various activities
- Provide coverage for the completion of Individual Care Plans (ICPs) and Interdisciplinary Care Teams (ICTs) when necessary
