Clinical Case Management Nurse

Position Overview
As a vital member of the Medix™ team, the Case Management Nurse will oversee daily operations and ensure the effective management of patient care plans. This role is essential in facilitating quality healthcare delivery and enhancing patient outcomes.
Location: Chatsworth
Schedule: 8:00 A.M. - 5:00 P.M.
Compensation: $50 - $55 per hour with Full Benefits
Key Qualifications
- Active California Registered Nurse License
- Minimum of 5 years of experience in Case Management, with a preference for Managed Care exposure
- At least 2 years of supervisory experience
- 2-5 years of experience in a Managed Care environment
- Proficient in Medicare Member Case Management
Core Responsibilities
- Maintain and manage the Master Log for daily staff assignments
- Ensure RN sign-off on all care plans completed from the previous day
- Delegate completed care plans to Case Management Coordinators for distribution
Biweekly Responsibilities
- Process payroll for the department
Monthly Responsibilities
- Compile Health Plan Monthly Reports
- Conduct Health Plan Monthly Meetings
- Facilitate Team Meetings
- Perform Internal Department Self Audits
Quarterly Responsibilities
- Prepare Health Plan Quarterly Reports
Annual Responsibilities
- Complete staff performance evaluations
Additional Duties
- Participate in Health Plan Webinars
- Engage in Health Plan Audits
- Maintain departmental tutorials, policies, and procedures
- Contribute to the development of new policies and procedures
- Involve in strategic planning initiatives
- Support various activities within the Case Management Department
- Provide coverage for the completion of ICPs and ICTs as needed
