Key Responsibilities

  • **Documentation:** Maintain accurate and Medicaid-compliant documentation of client progress and treatment plans.

  • **Education:** Educate families and groups on treatment and prevention programs.
  • **Communication:** Maintain effective communication with clients, families, and other healthcare professionals.
Requirements
  • **Education:** Master's degree in a licensable discipline (e.g. social work, counseling, psychology).
  • **Licensure:** Licensed Social Worker (LSW), Licensed Professional Counselor (LPC), or equivalent licensure.
  • **Experience:** Previous experience in case management, social work, or other related fields.
