Position Overview:

This role is dedicated to delivering essential custodial and personal care services to clients under the supervision of a Registered Nurse (RN) and in accordance with the established Plan of Care.

Educational Requirements: A High School Diploma or GED is mandatory; Current Nursing Assistant Certification with the state of Virginia and the Board of Nursing must be maintained in good standing.

Required Qualifications: Candidates must have successfully completed formal training and certification as a Nursing Assistant in compliance with Virginia state regulations. A minimum of one (1) year of experience in a home or long-term care environment is preferred.

Reporting Structure: This position reports directly to the Director of Nursing.

Key Responsibilities:

Ensure a clean, safe, and healthy living environment for clients. Responsibilities may include light housekeeping tasks such as tidying up bathrooms, organizing sleeping and living areas, and managing laundry and dishes. Execute all personal care tasks outlined in the written Plan of Care developed by the agency's Director of Nursing, which may involve assisting clients with personal hygiene, mobility, nutrition, dressing, grooming, and other assigned duties. Provide nutritional assistance as necessary. Monitor and report any changes in the client's health status. Perform additional activities as instructed and supervised by a Registered Nurse, which may include: Assisting clients with mobility aids such as wheelchairs, walkers, or hoyer lifts; Supporting prescribed range of motion exercises; Educating family members on household routines and skills; Tracking fluid intake and output; Measuring and documenting vital signs such as temperature, pulse, respiration, or blood pressure; Maintaining records of personal health care activities; Observing and reporting any significant changes in the client's appearance or behavior to the Registered Nurse. Assist clients with self-administered medication within the following guidelines: Retrieve medication from its designated storage in its original, labeled container and present it to the client. In the client's presence, read the medication label, open the container, dispense the prescribed dosage, and reseal the container. Provide the oral dosage directly to the client or assist them in taking it from another container. Return the medication container to its proper storage location. Document instances of assistance provided with self-administration of medication. Offer additional support for self-administration of medication as needed, which may include: Preparing necessary items such as beverages or utensils to facilitate the process; Assisting with opening medication containers or packaging; Supporting the client physically during the self-administration process as needed; Adhere to the agency's infection control protocols, including proper hand hygiene as per CDC guidelines. Follow all policies and procedures as outlined in the agency's Policy and Procedure Manual, Caregiver Handbook, and Caregiver Training Manual.
