Position Overview:
This role is responsible for delivering essential custodial and personal care services to clients who engage with Assisting Hands for support, under the supervision of a Registered Nurse (RN) and in accordance with the Plan of Care established by the RN.Educational Requirements: A High School Diploma or GED is mandatory; Current Nursing Assistant Certification with the state of Virginia and the Board of Nursing in good standing is required.
Required Qualifications: Candidates must have successfully completed formal training and certification as a Nursing Assistant in compliance with Virginia state law and regulations. A minimum of one (1) year of experience in a home or long-term care environment is preferred.
Reporting Structure: Reports directly to the Director of Nursing.
Key Responsibilities:
Ensuring a clean, safe, and healthy living environment for clients. Responsibilities may include light cleaning tasks such as tidying the bathroom, organizing sleeping and living areas, washing dishes, and laundry to maintain a hygienic and secure atmosphere for the client. Executing all personal care tasks outlined in a written Plan of Care as directed by the agency's Director of Nursing, which may involve assisting clients with personal hygiene, mobility, nutrition, dressing, grooming, physical transfers, and other assigned duties. Providing nutritional assistance as necessary. Monitoring and reporting any changes in the client's health status. Performing additional activities as instructed and supervised periodically by a Registered Nurse, which may include: Assisting clients with mobility aids such as wheelchairs, walkers, or hoyer lifts; Supporting prescribed range of motion exercises; Educating family members on household routines and skills; Tracking fluid intake and output; Measuring and documenting vital signs such as temperature, pulse, respiration, or blood pressure; Maintaining records of personal health care activities; Observing and reporting any noticeable changes in the client's appearance or behavior to the Registered Nurse. Assisting clients with self-administered medication, limited to the following tasks: Retrieving medication from its properly labeled container and bringing it to the client. In the client's presence, reading the medication label, opening the container, removing the prescribed dosage, and securely closing the container. Handing the oral dosage to the client or transferring it to another container to assist the client in taking the medication. Returning the medication container to its designated storage. Keeping a log of when assistance with self-administration of medication is provided. The CNA may also offer additional support with self-administered medication as needed by the client: Preparing necessary items such as juice, water, cups, or spoons to facilitate the self-administration process; Assisting with opening and closing medication containers or tearing foil from prepackaged medications; Providing support to the client during the self-administration process, such as steadying their hand or arm; Adhering to the agency's infection control protocols, including proper hand hygiene in accordance with CDC Guidelines. Complying with all policies and procedures outlined in the agency's Policy and Procedure Manual, Caregiver Handbook, and Caregiver Training Manual.
