Nurse Practitioner/Physician Assistant

Job OverviewWe are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments of older adults and other at-risk individuals in their homes, identifying chronic conditions and providing education and support to improve their overall health and well-being.
Responsibilities- Conduct adult/geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
- Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
- Collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up.
- Provide services in a variety of venues, including home visits, skilled facility visits, and telehealth visits.
- Master's degree required or commensurate experience and satisfactory completion of NP licensure.
- Current RN and NP or PA licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
- Board certification by the NCCPA, AANP, ANCC, or AACN in a Matrix-approved specialty.
- Minimum of 1 year experience as a Nurse Practitioner/Physician Assistant.
- Strong computer skills and familiarity with EMR software.
- Excellent verbal and written communication skills with patients, clients, and colleagues.
- Positive and outgoing personality, and ability to work as a team player.
- Ability to travel, valid state driver's license, and ability to drive a car, proof of adequate automobile insurance coverage for the state of residence.
- Travel required to meet patients where they live.
