Job Summary:We're seeking a skilled Registered Nurse (RN) to join our team. As a key member of our interdisciplinary team, you'll provide and arrange for Hospice care for patients and families in accordance with public health nursing standards, practices, and policies.
Responsibilities:- Initiate referrals to other disciplines and/or community resources as needed.
- Develop and implement preventive and rehabilitative nursing procedures to maximize quality of life.
- Collaborate with the Clinical Supervisor and Executive Director to conduct orientation, teaching, mentoring, and training of Hospice employees and volunteers.
- Participate in quality improvement activities, QUAPI program, and hospice-sponsored inservice training.
- Document care provided into the medical record, including clinical information, nursing services delivered, communications, and physicians' orders in a timely manner.
- Teach and supervise the teaching of family/household members/persons responsible for assisting the patient with their nursing or personal care needs.
- Deliver or supervise the delivery of skilled nursing services as outlined in the plan of care, with a focus on palliative, preventive, and rehabilitative nursing care.
- At least two years of public health/community health, med surg, or related nursing experience.
- Experience in home care, hospice, and/or oncology nursing preferred.
- Diploma or Associates degree in Nursing from an accredited nursing program.
- Healthcare Provider BLS (CPR) certification required.
This position requires consent to drug and/or alcohol testing after a conditional offer of employment is made, as well as ongoing compliance with the Drug-Free Workplace Policy.
