Home Health Physical Therapy Assistant

FeldCare Connects, a cutting-edge app-based network, is seeking a self-motivated Physical Therapy Assistant to provide exceptional home healthcare services to patients of all ages. This role is ideal for an Independent Contractor looking to serve the Stockton area specifically.

Your RoleAs a Physical Therapy Assistant in Home Health, you will be responsible for addressing patient care needs related to mobility, pain reduction, function restoration, and disability prevention. You will work closely with Physical Therapists to ensure treatment plans are followed, implement interventions to enhance patient wellbeing, and evaluate progress.
- Provide services defined by state laws governing physical therapy assistant practice, in accordance with the plan of care and in coordination with other healthcare team members.
- Assist patients in using therapeutic, creative, and self-care activities to improve functioning and safety.
- Observe, record, and report patient responses to treatment and changes in condition.
- Instruct patients, families, caregivers, and healthcare teams on areas of therapy where they can participate in assisting the patient.
To succeed in this role, you must possess:
- A strong ability and enthusiasm for working with diverse age groups.
- A valid Physical Therapy Assistant license and registration by the state.
- Completion of a Physical Therapy Assistant curriculum approved by The American Physical Therapy Association or equivalent organizations.
- Bilingual skills are highly valued.
