**Job Title:** Home Health Physical Therapist Assistant
Salary:$36-$45 per hour with a $3,000 signing bonus.
About Focus Health:We are a home health service provider enhancing the continuity of care for our patients. Our mission is to help them achieve their optimal level of independence through patient-centered care and evidence-based medicine.
Responsibilities:
- Perform various duties designed to improve strength and mobility as directed by a physical therapist.
- Help patients become independent by using therapeutic procedures to administer manual exercises, instructing, encouraging, and assisting patients with non-manual exercises, ambulatory functional activities, and daily living activities.
- Report significant changes in the patient's condition, safety concerns, unusual occurrences, and adverse effects of medical devices to the physical therapist or nurse manager promptly.
- Utilize physical agents, including heat, cold, water, light, electricity, ultrasound, and massage as directed in the plan of care.
- Observe, record, and report to the physical therapist, clinical manager, and healthcare provider, the patient's treatment and respond on a timely basis.
- Direct and aid patients in active and passive therapeutic exercises, muscle reeducation, gait and functional training, transfer activities, and prosthetic training.
- Provide and document services in accordance with applicable state, federal, Medicare, medical regulations, and standards, and third-party payer guidelines.
Requirements:
- Current license as a Physical Therapist Assistant in this state.
- At least one year of work experience under the supervision of a licensed physical therapist.
- Must have a vehicle, a valid current driver's license, and automobile insurance.
- Ability to make home visits and provide physical therapy evaluations and treatments in the home environment, complete documentation effectively, and submit promptly.
