Job Title: Occupational Therapist - Home Health

BAYADA Home Health Care is seeking a skilled Occupational Therapist to join our team in North Bergen, New Jersey.

Job Description:

We are looking for an experienced Occupational Therapist to provide high-quality care to our clients in their homes. As a member of our multidisciplinary team, you will work closely with nurses, therapists, and other healthcare professionals to develop and implement individualized care plans that promote independence, safety, and well-being.

Responsibilities:
  • Evaluate patients' functional status and develop personalized intervention plans to achieve favorable outcomes.
  • Conduct thorough assessments of patients' cognitive, psychological, and motor abilities, and identify equipment needs and environmental adaptations to enhance their quality of life.
  • Collaborate with the healthcare team to revise and update care plans as needed, and participate in multidisciplinary care planning to ensure comprehensive care.
  • Educate patients, family members, and caregivers on rehabilitative care and activities necessary to promote independence and safety.
  • Accurately document observations, interventions, and evaluations in a state-of-the-art electronic medical record system.
Requirements:
  • A current NJ Occupational Therapist license.
  • Prior home care experience preferred, but not required.
  • A minimum of one year of recent work experience as an Occupational Therapist.
  • Graduation from an occupational therapy program accredited by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal skills, organizational skills, and problem-solving skills.
  • Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
