Job Overview

We are seeking a compassionate Nurse Practitioner to provide evidence-based medical care for seniors and patients with complex needs. This role involves examining and treating chronic and urgent illnesses, collaborating with healthcare professionals, and managing comprehensive care plans.

Responsibilities

  • Evidence-Based Medical Care: Plan, direct, and evaluate patient medical needs by performing routine examinations, diagnosing illnesses, and prescribing treatment plans.
  • Patient Care: Treat chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses, ensuring timely and effective interventions.
  • Collaboration: Work with caregivers, healthcare professionals, and other interdisciplinary teams to develop comprehensive care plans, ensuring seamless transitions and coordinated care.
  • Documentation: Maintain accurate and up-to-date patient medical records, adhering to HIPAA regulations.

Requirements

  • Licensure: Active state Nurse Practitioners license and current National Board Certification.
  • Education: Master's degree in Nursing (MSN).
  • Experience: Five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
  • Skills: Strong interpersonal communication skills, excellent time management, proficiency in medical instruments and equipment, and knowledge of HIPAA and OSHA regulations.
