Job Summary

We are seeking a skilled Physical Therapist to join our Home Health team on a per diem basis. As a Physical Therapist with St. Tammany Health System, you will provide high-quality physical therapy services to patients in their homes, promoting independence and improving overall health outcomes.

Key Responsibilities
  • Develop and implement individualized physical therapy plans to meet the unique needs of each patient
  • Conduct thorough assessments and evaluations to identify patient goals and objectives
  • Provide hands-on therapy sessions, including exercises, stretches, and other interventions to improve mobility and function
  • Collaborate with other healthcare professionals to ensure comprehensive care and coordination
  • Document patient progress and outcomes in a timely and accurate manner
Requirements
  • Bachelor of Science degree in Physical Therapy from an accredited institution
  • Current licensure to practice physical therapy in the state of Louisiana
  • At least 2 years of experience in acute medical/surgical inpatient, inpatient rehabilitation, and/or outpatient rehabilitation settings
  • Current BLS certification through an ILCOR-approved course
Physical Demands

This role requires the ability to stand, sit, or walk for long periods, lift at least 50 pounds, and work in a moderate noise environment.
