About the Role

We are seeking a dedicated Registered Nurse to join our team as a Case Manager in Marion. This is an excellent opportunity for a passionate nurse to make a meaningful difference in patients' lives while advancing their career.

Job Summary

This role involves collaborating with colleagues to deliver high-quality patient care, ensuring compliance with best practices and promoting positive financial outcomes. You will work closely with interdisciplinary teams to assess, plan, implement, and evaluate patient care plans, focusing on health compliance and readmission prevention.

Responsibilities
  • Care Planning: Collaborate with healthcare teams to develop and implement individualized care plans, prioritizing patient-centered care and promoting optimal health outcomes.
  • Case Management: Utilize evidence-based guidelines to identify opportunities for cost containment, improve patient satisfaction, and enhance clinical outcomes.
  • Communication: Effectively communicate with patients, families, and healthcare providers to ensure seamless transitions of care and promote continuity.
Requirements

To be successful in this role, you will need:

  • A minimum of 5 years recent clinical nursing experience in relevant practice areas.
  • A Bachelor of Science in Nursing or related field.
  • Current licensure as a Registered Nurse with the North Carolina Board of Nursing or a compact state.
  • Nationally recognized certification in Case Management (ACM or CCM, or ANA approved certification).

Salary Range

The estimated salary for this position is $85,000-$110,000 per year, depending on experience and qualifications.
