Compassionate Hospice RN Case Manager

Gentiva Hospice is committed to delivering exceptional care and support to patients and their families. We are seeking a compassionate and dedicated Registered Nurse to join our Hospice Support Services team as a Hospice Registered Nurse Case Manager. In this role, you will provide direct patient care services and collaborate with an interdisciplinary team to ensure seamless coordination of care.
Key Responsibilities:
- Assess patient needs and develop comprehensive care plans in collaboration with the interdisciplinary team
- Provide direct patient care services, including medication management and wound care
- Communicate effectively with patients, families, and healthcare teams to ensure timely and accurate information exchange
- Participate in ongoing education and training to maintain current knowledge and skills
Requirements:
- Current RN license in good standing
- 1+ year of experience in hospice, home health, ICU, geriatric, or oncology nursing
- Strong assessment and problem-solving skills
- Able to work effectively in a fast-paced environment
Compensation:
- Salary range: $68,000 - $95,000 per year
