About the Role

VNA Health Group is seeking a skilled Physical Therapist to join our team in providing high-quality care to patients in their homes. As a Physical Therapist with our organization, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities
  • Assess and evaluate patients for physical therapy needs and develop individualized plans of care in collaboration with physicians.
  • Provide skilled physical therapy services to patients in their homes, including exercises, stretches, and other interventions to promote mobility and function.
  • Document all changes in patient condition and update care plans accordingly.
  • Identify counseling needs and plan appropriate teaching strategies to promote patient education and empowerment.
  • Monitor quality and appropriateness of services provided and adhere to agency infection control and safety policies.
Requirements
  • Physical Therapy Licensure in NJ required.
  • 2-3 years of prior clinical experience and community-based experience preferred.
  • Strong computer skills and ability to use electronic medical records.
  • Strong communication and interpersonal skills, with ability to work effectively with patients, families, and healthcare teams.
