Job Title: Home Health Physical Therapist

VNA Health Group is seeking a skilled Physical Therapist to join our team in North Arlington, New Jersey.

About the Role:

We are looking for a Physical Therapist to provide skilled nursing care to homecare patients in Newark, Livingston, Millburn, and Short Hills. The ideal candidate will have 2-3 years of prior clinical experience and community-based experience.

Responsibilities:
  • Assess and evaluate clients for physical therapy needs and develop and implement appropriate plans of action in collaboration with physicians.
  • Document all changes in clients' conditions and update care plans and outcomes accordingly.
  • Identify counseling needs and plan appropriate teaching strategies.
  • Monitor quality and appropriateness of services provided.
  • Adhere to agency infection control and safety policies.
  • Adhere to the organization's policy in regards to absenteeism and appearance.
Qualifications:
  • Physical Therapy Licensure in NJ required.
  • 2-3 years of prior clinical experience and community-based experience preferred.
  • Strong computer skills.
  • Strong communication and interpersonal skills.
  • Valid NJ driver's license.
