Community Integration Specialist

Job Summary
Community Options is seeking a dedicated and compassionate Community Integration Counselor to join our team. As a Community Integration Counselor, you will play a vital role in supporting individuals with disabilities to live independently and participate fully in their communities.
Key Responsibilities
- Support and encourage individuals to increase their ability to be as independent as possible.
- Meet with waiver participants on a regular basis to discuss their goals and progress.
- Collaborate with case managers to develop and implement personalized support plans.
- Provide ongoing support for community integration efforts, including transportation and accompaniment to medical appointments and community activities.
- Document individual progress and maintain accurate records.
- Ensure medical consultation forms are completed and medications are administered as needed.
- Coordinate leisure and volunteer opportunities to promote socialization and community engagement.
- Monitor health and medical needs, reporting any concerns to management.
- Drive individuals to medical appointments and activities in the community.
- Ensure program documentation and billable records are completed accurately and timely.
Requirements
- High School Diploma or GED.
- Bachelor's Degree preferred.
- Valid driver's license with a satisfactory driving record.
- Minimum 2 years of relevant experience supporting individuals with intellectual and developmental disabilities.
- Ability to establish a comfortable and supportive relationship with individuals receiving supports.
- Ability to exercise good judgment and remain calm in crisis situations.
- Excellent verbal and written communication skills.
- Excellent time management skills.
