Job Title: RN Case Manager

At Brooks Memorial Hospital, we are seeking a skilled RN Case Manager to join our team. As a RN Case Manager, you will play a critical role in ensuring the best possible outcomes for our patients.

Job Summary

The RN Case Manager will apply the Case Management body of knowledge to assess, plan, implement, coordinate, monitor, and evaluate options and services required to meet assigned hospital patients' health and service needs. This includes participating in and driving the utilization review process, negotiating the most cost-effective options available, and promoting quality and cost-effective interventions and outcomes for patients within a continuum of care.

Key Responsibilities
  • Assess patients' health and service needs, and develop comprehensive plans to meet those needs.
  • Participate in and drive the utilization review process to ensure the most cost-effective options are available.
  • Negotiate internal and external services to meet patients' needs, utilizing community and in-house resources for optimal patient outcomes.
  • Collaborate with physicians, patients, family, and other healthcare professionals to ensure seamless care transitions.
  • Develop and implement education plans to assist patients and families in understanding and carrying out medical recommendations post-hospital discharge.
Requirements
  • Current and unrestricted NYS Nursing license.
  • NYS Hospital and Community Patient Review Instrument (H/C PRI) Certification within 90 days.
  • New York State Department of Health SCREEN Certification within 90 days.
  • At least three years of recent clinical experience in an acute care setting.
  • Experience in discharge planning, case management, utilization review, and clinical pathways preferred.
Salary Range

$75,000-$89,000 annually. Wage will be determined based on factors such as candidate's experience, qualifications, internal equity, and any applicable collective bargaining agreement.
