Job Summary:

IntelyCare is seeking a skilled Licensed Practical Nurse (LPN) to join our team of healthcare professionals. As an LPN with IntelyCare, you will have the opportunity to work in post-acute facilities in the area, providing high-quality patient care and contributing to a safe and comfortable environment for residents.

Responsibilities:

  • Provide direct patient care, including administering medications, monitoring vital signs, and documenting health concerns and medical history
  • Collaborate with healthcare teams to develop and implement resident care plans
  • Conduct intakes, document health concerns and medical history of residents
  • Administer and properly verify medications and monitor their effects
  • Review residents' charts and medical records
  • Assist with the cultivation of a safe, comfortable facility
  • Maintain appropriate stocks of medical supplies
  • Participate in IV set up, immunizations, and phlebotomy procedures
  • Help with the orientation and training of medical staff

Requirements:

  • Current Illinois state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of approved Licensed Practical Nurse (LPN) educational program
  • High school diploma (or GED/equivalent)
  • Minimum of 6 months of experience (LPN or related role)
  • Ability to lift a minimum of 50 lbs
