We are seeking a skilled Social Worker to join our team at NYU Langone Hospital-Long Island. As a Per Diem Social Worker, you will provide psychosocial assessments and treatment to patients and families, assisting them in coping with hospitalization, illness, diagnosis, treatment, and life situations.
Key Responsibilities- Conduct thorough psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems.
- Develop and implement plans utilizing appropriate social work interventions, documenting findings and actions taken according to departmental and institutional guidelines.
- Treat and manage mental health and/or substance abuse problems, providing training and consultative services to other healthcare professionals.
- Arrange and coordinate patient and/or family pre and post hospital or treatment needs.
- Provide education to patients and families on adaptation to diagnosis, illness, treatment, and life situations, utilizing crisis intervention, brief and long-term individual, group, and family therapies.
- Participate in multi-disciplinary healthcare teams, providing leadership and representing the social work perspective to advocate for patients and families.
- Maintain records and statistics according to departmental and institutional guidelines.
- Master's Degree in Social Work.
- Current and valid New York State Licensed Master Social Worker (LMSW) license or Licensed Clinical Social Worker (LCSW) license.
- Years of social work or social work internship in a healthcare setting or social service agency.
- Solid working knowledge of social work practice, relevant medical and/or legal issues impacting patient care, and medical center resources to access and provide for patient care needs.
- Excellent communication, interpersonal, assessment, counseling, multitasking, detail orientation, and organizational skills.
- Basic computer skills, including Microsoft Office, especially Word, Excel, and PowerPoint.
