We are seeking a highly skilled Behavioral Health Therapist to join our team at Mid Michigan Health. As a key member of our primary care team, you will play a vital role in providing high-quality mental health services to our patients.
Key Responsibilities- Assess and manage patients with medical and behavioral health problems, working closely with our primary care providers to develop effective treatment plans.
- Provide brief behavioral interventions using evidence-based techniques, such as behavioral activation and problem-solving treatment.
- Use brief assessment tools to identify concerns, triage, and monitor patient outcomes.
- Document treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complications.
- Collaborate with our primary care team to develop and implement population health programs and behavioral change plans.
- Masters Degree in Social Work or related field.
- Licensure as a Licensed Psychologist or Master's Social Worker in the State of Michigan.
- Experience in casework principles, including diagnosis, assessment, crisis intervention, treatment, and transition planning.
- Knowledge of Integrated Behavioral Health practices, processes, and procedures.
This is a full-time position that requires regular and predictable attendance. The successful candidate will be able to work independently and as part of a team, with excellent communication and interpersonal skills.
