Job Title: Care Coordinator

Thrive Behavioral Health is seeking a highly motivated and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in supporting our clients in achieving their mental health and substance use recovery goals.

Job Summary:

The Care Coordinator will work collaboratively with our Access, Outreach, and treatment teams to provide support to clients, alleviate mental health and substance use distress, and eliminate access barriers to treatment. This role requires excellent communication and coordination skills, as well as the ability to multitask and work independently.

Key Responsibilities:
  • Outreach and coordinate with internal and external providers, specialists, community resources, and social service agencies to address client needs.
  • Track referrals, appointments, and follow-up activities in our electronic health record system.
  • Assist clients with gathering necessary personal identifying information and completing paperwork or applications for resource eligibility.
  • Enter progress notes, billing information, and data in a timely manner and in accordance with Thrive standards.
  • Research and stay updated on additional resources and supports available to assist Thrive clients.
Requirements:
  • High School diploma or GED required.
  • Associate degree in psychology, social work, or a human services-related field, or equivalent experience.
  • Minimum of one year of work experience in the behavioral health field or related social service field.
  • Excellent oral and written communication and coordination skills.
  • Ability to multitask and work independently.
