Nurse Case Manager

Mass General Brigham is seeking a skilled Registered Nurse to fill a Case Management position in Rollinsford, New Hampshire.
About the Role:This is an exciting opportunity to join our team as a Registered Nurse - Case Management. As a key member of our healthcare team, you will be responsible for coordinating patient care, ensuring seamless transitions between healthcare settings, and promoting optimal patient outcomes.
Key Responsibilities:- Facilitate coordination of care for patients along the healthcare continuum.
- Meet directly with patients and families to assess needs and develop individualized plans in collaboration with providers.
- Refer appropriate cases for social work interventions and collaborate with external case managers.
- Initiate and facilitate referrals for home healthcare, hospice, DME, and pharmacy services.
- Communicate and collaborate with all members of the healthcare team regarding patient needs, plans, and responses.
- 2+ years of experience in Med/Surg, Long Term Acute Care, or a similar field.
- Prior experience in Homecare, Emergency Department, or ICU is desired.
- Current and valid RN license.
- BSN and Case Manager Certification Training are preferred.
- Strong analytical, data management, and PC skills.
- Knowledge of Interqual, Milliman, and Midas.
- Excellent interpersonal communication and negotiation skills.
- Knowledge of third-party payer systems and levels of care.
- Current working knowledge of discharge planning, utilization management, case management, performance improvement, and managed care reimbursement.
