Job Title: Nurse Case Manager

Mass General Brigham is seeking a skilled Nurse Case Manager to join our team. As a key member of our healthcare team, you will be responsible for coordinating patient care and ensuring seamless transitions between healthcare settings.

Job Summary:

The Nurse Case Manager will work closely with patients, families, and healthcare providers to develop and implement individualized care plans. This role requires strong clinical assessment skills, effective communication, and a commitment to delivering high-quality patient care.

Responsibilities:
  • Assess patients for discharge needs and develop individualized care plans
  • Collaborate with healthcare teams to ensure timely and efficient care transitions
  • Communicate with patients, families, and healthcare providers to ensure informed decision-making
  • Document patient information and care plans in accordance with department standards
  • Participate in multidisciplinary discharge planning rounds and explore strategies to promote efficient resource utilization
Requirements:
  • 3 years of nursing experience
  • Experience in home health nursing preferred
  • RN degree required; BSN preferred
  • Certification in Case Management desired
  • Strong clinical assessment skills and knowledge of medical standards of care
  • Effective communication and interpersonal skills
  • Ability to work independently and as part of a team
