Job Summary

Theoria Medical is seeking a highly skilled Medical Director to lead our skilled nursing facilities (SNFs) across the country. As a Medical Director, you will be responsible for overseeing the medical operations of our SNFs, ensuring high-quality patient care, and collaborating with our Nurse Practitioners and Physician Assistants to manage day-to-day resident care.

Key Responsibilities
  • Serve as medical director for skilled nursing facilities, participating in administrative and quality initiatives
  • Attend and participate in monthly quality assurance and process improvement meetings
  • Provide guidance to facility staff, review and revise clinical guidelines as needed
  • Ensure compliance with state and federal regulations, train facility staff, and supervise clinical staff
  • Collaborate with Nurse Practitioners and Physician Assistants to manage day-to-day resident care
  • Provide high-quality, patient-centered, comprehensive primary care services by rounding on patients residing in SNFs
  • Diagnose and treat patients with acute illness, chronic diseases, and conditions
  • Document patient care in Theoria's proprietary electronic health record (EHR) system
  • Participate in Theoria's Chronic Care Management, Transitional Care, RPM, and other medical quality initiatives and programs
Requirements
  • Must be able to supervise collaborating nurse practitioners and physicians assistants
  • Must be able to utilize electronic/online medical tools, such as EHR systems and state/county medical systems
