Responsibilities- Interview, educate, and train clinical team members to ensure they have the skills and knowledge needed to provide exceptional care.
- Conduct client triage, including admissions, recertifications, and discharges, to ensure seamless care transitions.
- Collaborate with clients, the Clinical Director, and other care team members to develop and implement individualized Plans of Care that meet the unique needs of each child.
- Review documentation submissions to ensure accuracy, quality, and timely submission.
- Conduct home and/or hospital visits to assess client needs and provide support.
