About the Job
We are seeking a dedicated Care Navigator to join our team at San Andreas Regional Center. In this role, you will be responsible for supporting adult individuals with developmental disabilities to navigate the system and access the resources they need to live independently in their communities.

About the Role
The successful candidate will have a bachelor's degree in social work or a related field, as well as 2-3 years of experience in social work or case management. You will be responsible for conducting assessments, developing IPFs, and coordinating services with community resources. Strong communication and interpersonal skills are required, as well as the ability to conduct home visits and meetings in various settings. Fluency in Vietnamese is an asset, but not mandatory.

Responsibilities
- Conduct assessments and develop IPFs for adult individuals with developmental disabilities
- Coordinate services with community resources to support individuals in living independently in their communities
- Work closely with individuals, families, and other stakeholders to ensure that our clients receive the support they need
- Conduct home visits and meetings in various settings as required

Requirements
- Bachelor's degree in social work or a related field
- 2-3 years of experience in social work or case management
- Strong communication and interpersonal skills
- Ability to conduct home visits and meetings in various settings

Estimated Salary: $65,000 - $85,000 per year, depending on experience and qualifications.
