About the Role

Covenant Health (Tennessee) is seeking a highly skilled Care Coordinator to join our team at Fort Sanders Perinatal Center. As a Care Coordinator, you will play a critical role in coordinating the care of patients across the continuum of care, facilitating the achievement of quality, clinical, and cost outcomes.

Key Responsibilities
  • Assess and Coordinate Care: Identify patients' physical, psychological, and learning needs through consultation with healthcare providers and review of medical records.
  • Develop and Implement Care Plans: Assess and make recommendations regarding level of care placement based on patients' clinical status, home situation, and insurance benefits/personal resources.
  • Facilitate Communication: Improve continuity of care between all physicians involved in patient care, update referring physician offices on current patient status, and work closely with referring physician offices to ensure necessary information is transferred.
  • Connect Patients to Resources: Connect patients to relevant community resources to increase patient health, awareness, well-being, and satisfaction while helping reduce the cost of care.
  • Provide Primary Support: Be the primary contact for patients who have questions or need help, and participate in transdisciplinary team meetings to discuss and plan patient and family needs and discharge planning.
  • Monitor and Analyze Outcomes: Continuously monitor and report trends involving agreed-upon quality care issues, and acquire an understanding of the coding system to support and improve documentation.
  • Conduct Utilization Review: Conduct utilization review for identified patients within assigned service areas, and collect data concurrently for identifiable outcomes statistics as requested.
Requirements
  • Education: Minimum education requirement is not specified, but must be sufficient to meet the standards for achievement of the required license and/or certification.
  • Experience: Must have a minimum of three (3) years of clinical experience working within a women's services center.
  • Licensure: Current license in good standing with the State of Tennessee, MSSW/LCSW.
