- Develop and implement individualized service plans, focusing on consumer strengths, interests, and goals to encourage optimal utilization of available resources.
- Provide face-to-face contact with individuals and families in the Lower Bucks County community, building strong relationships and trust.
- Coordinate services provisions, including mental health services, substance use services, housing, and basic living needs, medical care, education, and employment.
- Participate in treatment planning meetings and provide support during crisis situations.
- Bachelor's degree in a related field, such as sociology, social welfare, psychology, or a related social science.
- Valid driver's license and ability to drive a company vehicle.
