Job Summary

We are seeking a skilled Licensed Vocational Nurse (LVN) to join our interdisciplinary team at Palomar Health Rehabilitation Institute. As an LVN, you will play a vital role in providing high-quality patient care and contributing to the success of our rehabilitation programs.

Key Responsibilities
  • Assess and Implement Patient Care
    • Conduct thorough assessments of patients to identify their needs and develop individualized care plans
    • Administer medications and treatments as prescribed by physicians and registered nurses
    • Monitor patients' progress and adjust care plans as needed
  • Collaborate with Healthcare Team
    • Work closely with registered nurses, physicians, and other healthcare professionals to ensure comprehensive patient care
    • Participate in interdisciplinary team meetings to discuss patient progress and develop care plans
  • Provide Education and Support
    • Educate patients and their families on disease management, medication use, and self-care techniques
    • Provide emotional support and guidance to patients and their families
Requirements
  • Graduate of an accredited Licensed Practical/Vocational Nurse program
  • Current state licensure as a Licensed Practical/Vocational Nurse
  • CPR/BCLS certification; ACLS preferred
  • IV Certified or obtained within 6 months highly preferred
  • Minimum six months' Medical/Surgical experience in an acute care setting preferred
  • Excellent oral and written communication and interpersonal skills
