Job DescriptionThis After-Hours Visit Licensed Vocational Nurse (LVN) position involves providing skilled nursing visits to patients and their families as requested by Intella Triage. You will be responsible for documenting observations and interventions related to the patient's environment, physical, mental, emotional, and social status, communicating care with other members of the Interdisciplinary Group (IDG) or team, including the patient's physician.

- Communicate care with other members of the IDG or team, including the patient's physician.
- Manage subcutaneous medication pumps.
- California LVN license.
- Preferred "IV Blood Withdraw" on discipline license.
- Graduate of an NLN accredited nursing school.
The estimated annual salary for this position is $75,000-$107,000, based on a 20-30 hour workweek at $35.70-$49.78 per hour.
