Overview:

The Medical Assistant Case Management Supervisor plays a pivotal role in overseeing the Patient Centered Medical Home (PCMH) care team, collaborating closely with the Manager of Medical Assistants and Case Workers.

Key Responsibilities:

  • Assist in the management of healthcare provider patient panels.
  • Contribute to the development of care initiatives aimed at assessing and coordinating population-specific healthcare needs.
  • Ensure accountability for both direct and indirect patient care for assigned populations.
  • Evaluate social determinants of health to address disparities in service delivery.

Duties Include:

  • Support the management of the PCMH Care Team in alignment with organizational policies to meet patient needs effectively.
  • Enhance patient engagement and team productivity by overseeing the assessment of provider patient panels, focusing on individual patient risk and population-specific requirements.
  • Facilitate the creation of comprehensive, collaborative care plans that align with provider treatment strategies and evidence-based chronic care guidelines.
  • Monitor population-specific risk management through registries and referrals.
  • Identify and address barriers when treatment objectives are unmet or care plans are not adhered to.
  • Track quality measures at both program and patient levels to devise interventions that enhance outcomes.
  • Act as a resource for community referrals within the practice.
  • Utilize the Electronic Health Record (EHR) system to document patient interactions and contribute to clinic workflows.
  • Manage daily responsibilities by reviewing system-related tasks and email instructions to ensure thorough case management.
  • Adhere to departmental policies, including accreditation standards and patient safety initiatives.

Qualifications:

  • Active Registered or Certified Medical Assistant.
  • Bachelor's Degree in public health, social work, or a related field is preferred.
  • Relevant experience in patient education, nursing, or case management may substitute for formal education.
  • Ability to recognize and respect cultural factors affecting patient choices and engagement.
  • Strong writing and communication skills, both verbal and written.
  • Capacity to work independently while managing priorities to meet deadlines.
  • Experience collaborating with multidisciplinary medical teams and familiarity with the PCMH model.
  • Proficiency in computer applications, including Microsoft Office and EHR systems.
  • Knowledge of medical documentation practices and privacy regulations.
  • Commitment to maintaining ethical and professional standards.
