Job Title: Youth Case Manager

Zepf Center is seeking a dedicated and compassionate Youth Case Manager to join our team. As a key member of our community-based services, you will play a vital role in providing support and coordination of services to individuals experiencing serious mental illness and/or addiction challenges.

Job Summary:

The Youth Case Manager will work with clients in the community, home environment, and at agency locations to provide a wide range of support and services. This includes assessing client strengths and needs, developing treatment plans, monitoring progress, and linking clients to additional community resources as needed.

Responsibilities:
  • Assess client strengths and needs with the client and applicable team members, conducts comprehensive care coordination, and develops treatment plans.
  • Monitors and evaluates client progress toward goal attainment. Reviews and modifies treatment plans as needed.
  • Maintains contact with the client as indicated in the treatment plan or as needed to ensure stability. Communicates with and provides support to clients in achieving goals and outcomes.
  • Provides active outreach as necessary, including crisis intervention and management, and acquisition of appropriate community supports.
  • Links and/or refers clients to other services, programs, and agencies as needed. Assists in assuring clients obtain and retain applicable entitlements and follows up on same.
  • Serves as an advocate for clients. With appropriate releases, communicates with family members and significant others as indicated.
  • Confers with supervisor and other team members regarding clients and, as appropriate, other agencies with emphasis on provision of most appropriate, effective, and cost-efficient services.
Requirements:
  • HS Diploma and/or GED equivalent
  • Three years of experience
  • Bachelor's degree or Master's degree
  • Valid Driver's License
  • QHMS certification required (at minimum) throughout employment
  • Social Work License and Mental Health experience preferred
  • SUD CM positions CDCA preferred
