**Job Description:**

We are seeking a highly motivated Healthcare Navigator to join our team at Prospect Medical Holdings. As a Healthcare Navigator, you will play a critical role in coordinating care for members enrolled in our outpatient clinical programs.

The estimated salary for this position is $58,000 - $68,000 per year, depending on experience and qualifications.

Key Responsibilities:

  • Screen incoming phone calls to determine urgency and direct calls to appropriate staff members.
  • Conduct telephonic outreach to members to discuss and offer outpatient clinical programs.
  • Request and gather necessary medical records to confirm program eligibility.
  • Coordinate care for members as appropriate, including scheduling appointments and creating authorizations.
  • May carry a caseload of clinically stable members for routine check-ins.

Qualifications:

  • Minimum 2 years of experience in case management and care coordination in an HMO environment.
  • High school diploma required; associates degree or higher preferred.
  • Proficient in Microsoft Office navigation, Excel, Outlook, Teams, Word, and other applicable systems needed for the role.
