Aveanna Healthcare is seeking a skilled Physical Therapist to join our dynamic home health care team. As a Physical Therapist, you will play a vital role in helping patients recover from illness or injury in the comfort of their own homes.
Key Responsibilities- Conduct initial and ongoing assessments to evaluate patients' mobility, coordination, strength, and current level of function.
- Administer, evaluate, and interpret tests and measurements to determine functional goals and outcomes.
- Develop and implement physical therapy plans of care, defining patients' physical therapy needs, goals, progress, and expected outcomes.
- Deliver physical therapy services in accordance with prescribed physician orders.
- Maintain accurate and timely documentation of patients' progress, modifying goals and treatment as needed, and determining discharge care plans as appropriate.
- Collaborate with healthcare teams, physicians, nurses, and other stakeholders to ensure seamless care coordination.
- Supervise and mentor other personnel, as applicable.
- Complete OASIS assessments, as allowed by state professional practice and regulation.
- Master's degree in Physical Therapy from an accredited university/program.
- Current, unrestricted state license as a Physical Therapist in the state of practice.
- Valid CPR and driver's license.
- Strong organizational and communication skills.
- Attention to detail.
- Effective time management.
- Proven relationship-building skills.
- Experience in home health therapy and completion of OASIS documentation.
