Job Title: Home Health Physical Therapist Full Time

Position Overview

Aveanna Healthcare is seeking a skilled Physical Therapist to join our dynamic home health care team. As a Physical Therapist, you will play a vital role in helping patients recover from illness or injury in the comfort of their own home.

Key Responsibilities:

  • Conduct thorough assessments to evaluate patients' mobility, coordination, strength, and current level of function.
  • Administer, evaluate, and interpret tests and measurements to determine functional goals and outcomes.
  • Develop and implement personalized physical therapy plans to achieve optimal patient outcomes.
  • Collaborate with healthcare teams, physicians, and caregivers to ensure seamless care coordination.
  • Document patient progress, modify treatment plans as needed, and determine discharge care plans.
  • Supervise and educate patients and their caregivers on home exercise plans to ensure successful transitions.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.

Requirements:

  • Degree in Physical Therapy from an accredited university/program.
  • Current, unrestricted state license as a Physical Therapist in the state of practice.
  • Valid CPR and driver's license.

Preferred Qualifications:

  • Strong organizational and communication skills.
  • Attention to detail and time management skills.
  • Proven relationship-builder with a focus on patient-centered care.
  • Experience in home health therapy and OASIS documentation.
